You can call the member services number on the back of your insurance card and ask:

1. Do I have out-of-network benefits for outpatient therapy?

2. What is my out-of-network deductible? 

3. What percentage is covered for out-of-network therapy once I’ve hit my deductible? 

4. My therapist’s CPT code is 90791 (intake code), 90837 (individual session code), and 90847 (couples or family code) what will be reimbursed for my 53 minute psychotherapy session?

5. Can you please instruct me on how to submit my claim for reimbursement?
